Your Application for Tutoring Services:
On this page are links to two application forms.
Our first form is our Scholarship Application form. We seek funding from a variety of donors and grants to support children, students, and learners. Donations provide our need-based student scholarships for specialized reading tutoring. We use a sliding fee based on the parent(s) or guardian(s) income. If you need help completing the form, please contact us, so we can help you. If you have a disability and need accommodations, please let us know, so we can accommodate you. We care about you and your child or student! We sincerely want to help. We look forward to talking with you!
Our second form is our Self-Pay Application form. Parent(s) or guardian(s) who are able to pay for their child’s or learner’s tutoring use this form to provide us with the information we need to consider if our services are appropriate for your child or learner. We use a set fee structure for our self-pay clients which is designed to help us conserve our resources as we reach and teach as many students as possible, including your child. If you need help completing the form, please contact us, so we can help you. If you have a disability and need accommodations, please let us know, so we can accommodate you. We care about you and your child or student! We sincerely want to help. We look forward to talking with you!
